Summary Case Report: A 37-year-old semi-truck driver was driving on an interstate highway at 4:40 am. He had pulled onto the right shoulder and was outside the truck at the left rear apparently checking the brakes for a possible fire. A second semi-truck came from behind and struck the first truck on the left side, killing the victim and dragging him down the highway. The second truck trailer overturned with the victim underneath it.

Summary Case Report: A 48-year-old farmer was moving grain on a neighbor's farm. He was working with a power take off unit (PTO) connecting a tractor and a grain wagon, when he got caught up in the PTO, which was unshielded. He was found dead at the scene. All his clothing was entangled in the PTO. He was found underneath the PTO: his right arm had been torn off at the shoulder.

Summary Case Report: A 43-year-old welder was one of two men working on the side of a high-rise building, 19 stories high. Both men were suspended in separate metal work cages suspended by 1/2 inch cable. Apparently the electrical wire feeding the victim's arc welder shorted out and arced to the adjacent steel cable and melted the cable sending the cage and the worker to the ground. Summary Case Report: A 44-year-old independent truck driver was loading a new hydraulic road grader onto a flat bed "belly trailer" for delivery to customer. The road grader was backed onto the trailer and the victim was in process of securing the grader with chains. He was working alone. He was found pinned between the right rear tire of grader and the sidewall of trailer – asphyxiated. Victim was obese and apparently slipped and wedged himself in the space between the wheel and the sidewall.

Summary Case Report: A 41-year-old truck driver for an excavating crew was driving a large dump truck along a roadside terrace. After coming down a hill he failed to maneuver a turn and went straight off the terrace and overturned the truck. He was thrown from the truck and was crushed under one of the wheels and died instantly. Brake failure has been ruled out. He was reportedly "driving erratic that day" for an unknown reason. All medical tests were negative.

Summary Case Report: A 45-year-old construction worker was part of a safety crew sent to secure a hole in the roof of a new hotel. He apparently slipped and fell through the hole to the concrete surface of a floor 20 ft. Below him. He received massive head and chest injuries and was flown by helicopter to a regional hospital where he died. His wife had filed for divorce the day before his death.

Summary Case Report: A 33-year-old worker for a construction company was helping to erect a steel building in the winter. He was standing on steel roof beams 25 ft. High welding steel bridging between steel roof joists. He was wearing a safety belt and lanyard. He unhooked himself to reposition for another weld and slipped off the roof beam, hitting his head on the joist. He fell motionless to the frozen ground and received fatal head injuries.

Summary Case Report: A 26-year-old man was helping a co-worker bury sewer pipes in a 10 ft. Trench. They were using a backhoe and a 20 ft. Steel safety box in the trench. They were trying to pry up a slab of concrete with the backhoe scoop when it slipped sideways. Momentum of the scoop pinned the victim to the side of the trench box causing massive internal injuries and he died 1 hour later. The backhoe did not malfunction. Summary Case Report: A 61-year-old farmer was helping to unload corn into a truck from an overhead corn bin. He was shoveling shelled corn off a ledge inside the bin when he apparently slipped and fell into the corn. He suffered a heart attack trying to get out, but died of asphyxiation, according to coroner's report. He was alone in the bin at the time, with an assistant working outside. He became engulfed in the moving grain and was not discovered until the flow of grain stopped.

Summary Case Report: A 30-year-old semi-truck driver was hauling tanker of anhydrous ammonia to distribution site for an agricultural fertilizer company. He lost control of his truck while rounding a downhill curve on a primary road and hit a rock bluff and tipped over on its side, rupturing the large ammonia tank. The truck was immediately engulfed in ammonia fumes which continued for 2 more days. The man died quickly from asphyxiation inside the cab.

Summary Case Report: A 61-year-old farmer was using a tractor with front end loader to push down a tree that had partially blown down last winter. Using the loader bucket he pushed on the tree, it came loose from its base and fell towards the tractor, knocking the man backwards off his tractor seat. The man received massive internal chest injuries and died within a few minutes. His son saw the accident and called the ambulance.

A co-worker warned him when he was about to restart the belt from a remote location, but the victim was still trying to clean the belt. When the belt started, he fell down on the belt, rode 150 ft. He loosened the bolt on the tire but could not get it off. He then loosened the hydraulic coupling to the cylinder for that wheel, hoping it would relieve pressure on the wheel, but fluid shot out and the right side of the planter fell on the man and crushed him. Summary Case Report: A 57-year-old maintenance worker for an industrial management company was helping to install a new boiler. He was walking next to a concrete pit which was covered with 2 x 4s and plastic to cure the new concrete. He apparently dropped something on the plastic, then stretched out on the wooden frame to retrieve it. Summary Case Report: A 19-year-old male driving a tricycle tractor dropped right rear tire off a gravel road and rolled the tractor in a ditch. The tractor had a canvas heater housing which made escape impossible. Victim was pinned only by his ankle, but received large penetrating wound to his upper leg / groin area which severed his femoral artery. Victim quickly bled to death. Wound probably came from iron structure of heater housing.

Summary Case Report: A 20-year-old sheet metal worker was on top of a metal building putting insulation on top of ceiling sheet metal in preparation for roofing with heavier steel. Man apparently slipped off a purlin and landed on a ceiling seam which broke open. Several screws snapped off and he fell through the opening 21 feet to concrete surface below. Summary Case Report: A 45-year-old foundry maintenance worker with 23 years experience was repairing an electric arc furnace. He was using water to cool the spout on the furnace, when water entered the furnace causing a steam explosion in the slag eliminator and spout. He suffered 2 and 3 degree burns on his upper body--was taken to a regional hospital burn unit. He died from aspiration while in the hospital getting ready for a skin graft.

Summary Case Report: A 25-year-old sheet metal journeyman was working on an elevated catwalk at an electrical generating station. He dropped something to a level below and used a rope to climb down to retrieve it. He could not get back up so a co-worker lowered a ball and chain from a crane to pull him up. While stepping off of the ball, the victim lost his grip or his footing and fell 65 feet to the ground. He died from neck injuries.

Summary Case Report: A 66-year-old farmer was driving a tractor pulling a grain train wagon. He turned off a state highway into a field driveway and had to cross train tracks which were parallel to the highway. A railroad evaluation car was coming at 40 mph down the track and hit the tractor broadside. The railcar blew its horn and set brakes, but farmer didn't notice. The crossing was unmarked. Tractor had a cab and the farmer had good hearing. The ground was flat without obstructions.

Summary Case Report: A 39-year-old hydraulic mechanic was sent to repair a hydraulic man-lift which had jammed in the up position at a warehouse construction site. Man was underdressed for the -40 wind chill and probably in a hurry. He was found pinned under the lower extension arm, on his knees, with boom arm on his back. Hydraulic fluid was on the victim and the floor. He did not use lock-out pins before he began work. He was dead at the scene.

Summary Case Report: A 51-year-old switchman for a freight railroad was working in the switchyard at an industrial location, doing his normal work duties. He was found dead on the tracks after being run over by a railroad car. The accident was unwitnessed, and no one knows if he fell off the train or tripped on the ground and fell on the tracks. No abnormal work conditions existed that day. The case was investigated by Iowa Department of Transportation.

Due to heavy rains the bus got stuck in high water and mud on a road. Everyone got out of the bus and waiting for help to arrive. Summary Case Report: 74-year-old transfer driver for a truck rental company was driving on an an interstate highway through a construction zone where the road was reduced to 2 lanes. A collision between an auto and another semi-trailer occurred in front of the victim, and his semi-trailer crashed into them. He fell 16 feet off the edge of the building and landed on his head on a concrete slab below. He was unconscious when found and died one hour later in the hospital from massive head injuries. The general contractor was from out-of-state. OSHA obtained restraining order to stop construction at this location.
